(a) Location - 175 Broadway Malden, MA, 02148  On or about 9/19/2012, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2012.  1. 3/2/2012, lifting a window on a machine developed a cramp in abdomen and back.  2. 6/14/2012 lifting bags of trash, pulled muscle in neck and side.  3. 7/10/2012, an employee injured a hand and foot when a stone slipped out of the rigging while hoisting it.
