29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): (a) G.E. Construction, Inc., @ 29th Street & Taft Ave., Loveland, CO 80537: On and before August 30, 2011, the exposing employer did not ensure that each employee performing activities in the unprotected vertically- walled excavation was protected from a possible cave-in during an installation of a concrete vault. The unprotected vertically sided excavation measured appropriately 6' 6 deep, 6wide, and 18 feet long and lacked any protective system, such as appropriate sloping, benching, or a shield system. This condition exposed employee crushing hazard by excavation wall cave-in. (b) G.E. Construction, Inc. was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.652(a)(1), which was contained in OSHA Inspection Number 978338, Citation 1, Item 1 issued on August 26, 2011 and became Final Order on October 6, 2011, with respect to a workplace located at West Eisenhower Blvd & Wilson Ave., Loveland, CO 80537. Abatement Note: All excavations 20 feet or less in depth which have vertically sided lower portions shall be shielded or supported to a height at least 18 inches above the top of the vertical side. All such excavations shall have a maximum allowable of slope 1 ? to 1.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) G.E. Construction, Inc., @ 29th Street & Taft Ave., Loveland, CO 80537: On and before August 30, 2011, the exposing employer did not ensure that each employee performing activities in the unprotected vertically- walled excavation was protected from a possible cave-in during an installation of a concrete vault. A ladder, ramp or other safe means of egress was not provided for the employees working in the vertically-walled excavation. The means of entry and egress were ascending and descending excavation ends and climbing up the side walls of the excavation. This condition exposed employee to crushing hazard by excavation wall cave-in.
