29 CFR 1926.451(f)(7):     Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.      Worksite:  The employer did not ensure that the pump jack scaffolding had been inspected by a competent person prior to allowing employees to begin work on it.

29 CFR 1926.454(a):     The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.        Worksite:  The employees who worked from pump jack scaffold without fall protection at heights of approximately 28 feet 9 inches above the ground were not trained to recognize the hazards associated with the type of scaffold being used.

29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.      Worksite:  An employee was exposed to fall hazards of approximately 28 feet 9 inches while working on a pump jack scaffold without fall protection.

29 CFR 1926.1053(b)(16):     Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.      Worksite:  Employees were using an aluminum ladder with two broken feet which had not been removed from service until repaired or replaced.
