29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets:    a) Skyline Roofing and Sheet Metal Company, Inc., Toledo, Ohio:  On or about August 31, 2012, at the address of 1525 E. Alexis Rd., Toledo, Ohio, the employer did not ensure that each employee engaged in roofing activities on a commercial structure with a flat roof were protected from falls.  No acceptable form of fall protection was being utilized to protect employees cleaning debris from falling approximately 11.3 feet to the concrete ground below, which could result in permanent disability or death.     Skyline Roofing and Sheet Metal Company, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA Inspection # 314848755, Citation number 1,  Item number 2, and was affirmed as a final order on March 10, 2011 with respect to a workplace located at Corner of University and Broadway, Urbana, IL 61802.
