29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): a) Parkway Cleaners: An employee, (Stain Remover) while removing stains from clothing used products such as, but not limted to, Pyratex which contains aliphatic carboxylic ester, glycol ether and aliphatic ketone as well as other stain removers such as ammonium hydroxide. The employer did not devleop or implement a written hazard communication program to address how material safety data sheets, labeling and training would be addressed at the site. On or about August 31, 2012.

29 CFR 1904.7(a): The employer did not record an recordable injury resulting in restricted work and days away from work: a) Parkway Cleaners: An employee, (Washer) while walking into the Parkway Cleaners door tripped and fell on June 4, 2012 injuring the right shoulder which resulted in days away from work as the employee needed surgery to repair the shoulder. The employer did not maintain OSHA 300 injury and illness logs on which the injury would be recorded. On or about September 22, 2012.
