29 CFR 1926.451(e)(8):     Direct access to the roof from the scaffold was not adequately provided to comply with this part:    An employee used the scaffold platform which measured approximately 22 feet, 6 inches to access the roof landing which measured approximately 25 feet, 8 inches.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.454(a)(2):     The employer did not have each employee who performed work on a scaffold trained by a qualified person in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control those hazards.  The training did not include fall hazards, falling object hazards, and proper scaffold use:    Employees working from a tubular welded scaffold did not receive training from a qualified person to include fall hazards, falling object hazards, and proper scaffold use.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Fall protection systems were not in use as an employee was working on a roof approximately 25 feet, 8 inches above the ground cleaning mortar off old bricks and stacking bricks.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
