The employer was excavating to 8 ft deep in the sidewalk to get to a sewer pipe. The employer encountered other utility (pipe) and rocks on one side resulting that the excavation was too narrow to fit a ladder and the employee in it at the same time. The employer failed to make the hole wider/ failed to provide a ladder.    Location: 162 E 64th St, New York, N.Y.
