29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a) Workers were observed using Portland cement for the repair of an exterior wall with no eye or skin protection exposing them to a chemical exposure hazard.

29 CFR 1926.404(b)(1)(ii):  Ground-fault circuit interrupters. All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, shall have approved ground-fault circuit interrupters for personnel protection. Receptacles on a two-wire, single-phase portable or vehicle-mounted generator rated not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters.    a) A worker was observed, using a damaged extension cord, plugged into an outlet without a cover, with no GFCI.

In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    a) A worker was observed, using a damaged extension cord, plugged into an outlet without a cover.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.    a) Two workers were observed on a scaffold which was not properly guarded by railings exposing the employees to a 14 foot fall hazard to the ground below.
