29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    (a)  6375 Carlisle Pike, Mechanicsburg Pennsylvania - Employees worked on the roof with an approximately 4 and 12 pitch without fall protection.   The employer did not ensure that employees used the fall protection, on or about August 21, 2012.      No Abatement Certification or Documentation Required.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.    (a) 6375 Carlisle Pike, Mechanicsburg Pennsylvania - The forks of a Ingersoll Rand high reach were left elevated with no employees at the controls or within 25 feet, on or about August 21, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operators performance was not conducted at least once every three years (as referenced in 29 CFR 1926.602(d))    (a)  6375 Carlisle Pike Mechanicsburg Pennsylvania - An employee operated a fork lift to elevate materials to the roof.  The employer did not ensure that the employee was provided with training at least once every three years, on or about August 21, 2012.
