Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. (a) 6375 Carlisle Pike, Mechanicsburg Pennsylvania - Employees worked on the roof with an approximately 4 and 12 pitch without fall protection. The employer did not ensure that employees used the fall protection, on or about August 21, 2012. No Abatement Certification or Documentation Required.
