29 CFR 1926.552(b)(2)(i): Bars on hoistway entrances were not located 2 feet from the hoistway line: a) All Hoistway Entrances - The bars on the hoistways were located flush against the exterior walls only 11 inches back from the hoistway centerline. Carpenters were observed signaling the hoist car by leaning out of the hoistway opening where the guardrails were installed only 11 inches back from the hoistway centerline exposing the carpenters to struck-by hazards.

A mason foreman and laborer were observed standing directly under a material hoist car which was not provided with a 6 ft. ground barrier exposing them to a potential struck by hazard.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail: a) West Side, Rear of the Building - A superintendent was observed using the exterior stairs, 8 risers high, as access into the building, exposing employee to a 6 foot slip, trip, or fall hazard. b) Throughout the Building - A superintendent, assistant superintendent and workers were using interior stairs, 15 to 19 risers high, to access the various work areas exposing the employees to slip, trip or fall hazards of 12 feet where the stairs were not equipped with a standard handrail.
