29 CFR 1926.552(b)(2)(i): Bars on hoistway entrances were not located 2 feet from the hoistway line: a) East Side of the building on all floors - The bars on the hoistways were located flush against the exterior walls only 11 inches back from the hoistway centerline. Carpenters were observed signalling the hoist car by leaning out of the hoistway opening where the guardrails were installed only 11 inches back from the hoistway centerline exposing the carpenters to struck-by hazards.

A mason foreman and laborer were observed standing directly under a material hoist car which was not provided with a 6 ft. ground barrier exposing them to a potential struck by hazard.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail: a) Internal Stair Systems throughout the Building - Stairways were not equipped with at least one handrail. Employees were observed using interior stairs for accessing the various work areas where the stairs were not equipped with a standard handrail. Each stairway Contained 15 to 19 risers exposing the employees slip, trip or fall hazards of 12 feet. b) Primary Entrance into the Building - Stairways were not equipped with at least one handrail. All employees were observed using the exterior stairs as access into the building, they contained 8 risers exposing them to a 6 foot slip or trip hazard.
