29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Rayco Building, Roof -- On or about August 30, 2012, two employees were walking on the roof of a one-story, low sloped, auto mechanic building without a guardrail system, safety net system, or personal fall arrest system in place, exposing employees to a fall hazard of no less than 11 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Rayco Auto Mechanic Building, Roof -- On or about August 30, 2012, the rails of a portable extension ladder used by employees for access to a roof did not extend 3 feet above the upper landing surface and was not secured at its top to a rigid support. Two employees used this ladder to access the roof of a one-story auto mechanic shop, exposing employees to a fall hazard of no less than 11 feet.
