At the site located at 6060 South Loop East, Suite # 110 in Houston, TX; the employer failed to maintain OSHA 300 Logs. An employee sustained injuries on 8/3/2012 when he fell in the rear loading dock area and required medical attention to treat injuries, these injuries should have been recorded on OSHA's 300 logs or equivalent forms.
