29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access: a) A foreman and a mason were observed on a tubular welded frame scaffold (TWFS), climbing the cross braces and poles to access different levels of the scaffold exposing them to a 19 foot fall hazard onto the debris covered concrete side walk below.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a) Southeast Corner of the Building - Guardrail systems were not installed along all open sides and ends of platforms. A mason was observed on a tubular welded frame scaffold without adequate fall protection exposing him to a 19 foot fall hazard onto the debris covered concrete side walk below.

A mason foreman and laborer were observed standing directly under a material hoist car which was not provided with a 6 ft. ground barrier exposing them to a potential struck by hazard.
