29 CFR 1910.24(f):     Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs:    a) Upstairs Tobacco Zone 62 and 52 - An eight foot high flight of steps leading to a work area on the upper level included a top step that was 2 3/4 inches higher than the rest of the steps, thereby exposing the employee to a trip hazard, observed on or about 9-3-12.    Abatement certification required.

29 CFR 1910.176(a):     Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage must be made:    (a) Zone 41 Chocolate Room Location 74-423702 - An employee used a Crown RR5000 reach fork lift to place and pull pallets from shelves in the aisle where support I beams restricted the operators ability to safely move the truck and restricted clearance between the truck, the I beam, and the shelves, thereby exposing the operator to a struck against and/or caught between hazard, observed on or about 9-03-12.
