29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) Jobsite - 4474 Silverwood Street - Employees working on the tubular welded frame scaffold were exposed to fall hazards of 13 feet without the use of a guardrail system or personal fall arrest system.  Employees were working on the 2nd level of the scaffold on or about 8/29/12.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement and states employees and their representatives were informed of this abatement.

29 CFR 1926.454(a):The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a) Jobsite - 4474 Silverwood Street- The employer did not provide each employee performing stucco operations with training to recognize the hazards and the procedures to control the hazards associated with scaffolds, on or about 8/29/12.       Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    Jobsite - 4474 Silverwood Street - A ladder or other safe means of access was not provided to employees working on the tubular welded frame scaffold.  Employees were using the side frames and cross bracing to access the working level. Obsreved on or about 8/29/12.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:    a) Jobsite - 4474 Silverwood Street - The working levels of the tubular welded frame scaffold were not fully planked between the front and rear uprights.  Employees were performing stucco operations on the 2nd level of the scaffold.  Observed on about 8/29/12.       Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement and states employees and their representatives were informed of this abatement.
