29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. Job site located at 930 Main Ave., Crete, NE 68333-The employer used a computer where the power source was a electrical cord plugged into the power outlet in the shipping/receiving area. The electrical cord being used to provide the electrical power for the computer was missing the grounding prong for the plug-in exposing employees who used the computer to an electrical shock or electrocution hazard.

29 CFR 1910.1200(g)(11): Material safety data sheets were not made readily available, upon request, to designated representatives and to the Assistant Secretary, in accordance with the requirements of 29 CFR 1910.1020(e). The Director shall also be given access to material safety data sheets in the same manner. Job site located at 930 Main Ave., Crete, NE 68333-The employer was using cleaning products from the Pamida store that they as Shopko had just bought out. The employees were using the Pamida chemicals and the store did not have Material Safety Data Sheets on the site for the chemicals. The employer did however have chemicals that Shopko had sent to the store and the employer did have the MSDSes for the Shopko products. The employees were still using the Pamida products without having a MSDS for the products.
