29 CFR 1926.451(f)(6):     Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).  In this case the voltage was less than 300 volts and required 3 feet minimum clearance.     (a) Location)  88 Endicott Street Peabody, MA, 01960  On or about 8/24/12, a pump jack scaffold was erected where the scaffold came within the approach distance for an energized power line.
