Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to chemical burns and inhalation hazards from an uncontrolled release of anhydrous ammonia: a) Warehouse Ammonia Refrigeration System, on the rooftop, on or about 8/31/12: The employer did not ensure ongoing mechanical integrity of the piping in the anhydrous ammonia refrigeration system. Damaged insulation and visibly corroded pipes were observed during the inspection. Abatement documentation must be submitted for this item
