29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment shall be consistent with applicable manufacturers recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience. a) On the roof, and throughout the facility, on or about 9/20/12: The employer did not conduct annual inspection of process pipes, where pipe insulation was missing or broken, in accordance with frequency of RAGAGEP's, IIAR Bulletin 110 for an ammonia refrigeration system containing approximately 14,600 pounds of anhydrous ammonia. b) Throughout the facility, on or about 9/20/12: The employer did not perform annual inspections of process equipment in accordance with RAGAGEP's, IIAR Bulletin 110 for an ammonia refrigeration system containing approximately 14,600 pounds of anhydrous ammonia. Abatement certification must be submitted for this item.

29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before further use or in a safe and timely manner when necessary means are taken to assure safe operation. For an ammonia refrigeration system which contained approximately 14,600 pounds of anhydrous ammonia: a) On the roof, in hallways, and in freezer and cooler rooms, on or about 9/20/12: Process piping did not have contents or flow direction labels b) On cooler unit 11, and freezer units 3,4,9, and 10, on or about 9/20/12: Pressure vessels were not equipped with pressure relief devices. c) Outdoors near condensers and in hallways next to freezer and cooler rooms, on or about 9/20/12: Sections of process piping had missing insulation and visible rust and/or ice buildup. d) On the roof, on or about 9/20/12: Safety relief valves on pressure vessels did not discharge at a height of at least 15 feet above roof level. e) On pressure vessel V3, and in hallway outside freezer 10, on or about 12/5/12: Ice had accumulated on valves so that they could not be turned. Sodus Cold Storage Co Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 310748132, citation number 001, item 005 and was affirmed as a final order on October 18, 2007, with respect to a workplace located at 50 Maple Avenue, Sodus, NY 14551.
