29 CFR 1910.26(c)(2)(vii): Ladders having defects were not marked and taken out of service until repaired. On or about September 04, 2012, an employee was using a Louisville, 40-foot extension ladder that was extended approximately 21 feet to repair the Lithonia ballast, enclosed Low Bay lamp with light bulb. The ladder was missing the rope to disengage the locking mechanism, exposing the employee to the hazard of being struck by the extended ladder. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders having defects are marked and taken out of service until repaired.

29 CFR 1910.333(c)(7): Portable ladders shall have nonconductive side rails if they are used where the employee or the ladder could contact exposed energized parts. On or about September 04, 2012, an employee was working from a 40-foot aluminum extension ladder extended to 21 feet, repairing a Lithonia ballast, enclosed Low Bay lamp with light bulb. The ladder was aluminum and was not equipped with non-conductive side rails, exposing the employee to an electrical shock hazard. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps to ensure that portable ladders will have nonconductive side rails if they are used where the employee or the ladder can come into contact with energized electrical parts.

The light switch was in the "on" position and the circuit was not de-energized, exposing the employee to an electrical shock hazard. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that safety related work practices are used to protect employees who are exposed to the electrical hazards involved.

On or about September 04, 2012, an unqualified employee was working from a 40-foot, aluminum extension ladder extended to 21 feet, repairing a Lithonia ballast, enclosed Low Bay lamp with light bulb. The employee was not qualified to work on the 480-Volt light that was energized, exposing the employee to an electrical shock hazard. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that only qualified persons are working on energized circuits and that they are familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools.
