29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide. a) North-West Side of Building, 3rd Section - A mason was observed working from the third level of a tubular welded frame scaffold which was not fully planked. The employee was exposed to a fall from elevation of greater than 14 feet to the ground below. b) North-East Side of Building, 2nd Section - A mason was observed working from the 2nd level of a tubular welded frame scaffold which was not fully planked. The employee was exposed to a fall from elevation of greater than 13 feet to the ground below. c) North-East Side of Building, 1st Section - A mason was observed working from the 1st level of a tubular welded frame scaffold which was not fully planked. The employee was exposed to a fall from elevation of approximately 6 feet - 8 inches to the ground below.

This condition exposed employees to a fall from elevation of greater than 13 feet to the ground below. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a) North-West Side of Building - Masons were observed using the cross braces for access and egress to their platform on a tubular welded frame scaffold. This condition exposed employees to a fall from elevation of greater than 20 feet to the ground below. b) North-East Side of Building - A mason was observed using the horizontal members of the tubular welded frame scaffold for egress. This condition exposed employees to a fall from elevation of greater than 13 feet to the ground below.
