29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    a.  On or about August 30, 2012, employees cutting wood on the ground and throwing wood pieces to the carpenters doing the roof sheathing of a residential house were not wearing hardhats, exposing themselves to head injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    a.  On or about August 30, 2012, two employees installing roof sheathing on a 7:12 pitch residential roof were not being protected by a guardrail system, safety net or personal fall arrest system, exposing themselves to a 10.5 feet fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a.  On or about August 30, 2012, in the North West side of the residential roof, an employee doing roof sheathing on a 7:12 pitch roof set up the Louisville 20-foot fiberglass extension ladder at 2 feet above the landing surface, exposing himself to a 10.5 feet fall hazard.     b.  On or about August 30, 2012, in the South West side of the residential roof, an employee doing roof sheathing on a 7:12 pitch roof set up the Werner 24-foot aluminum extension ladder at 1 ? feet above the landing surface, exposing himself to a 10.5 feet fall hazard.
