29 CFR 1910.132(d)(1)(i):  When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard.    (a)   Spray booth;  on or about August 22, 2012, and at time prior thereto, the employee spraying a truck bed with Ameriguard coating wore only a half mask respirator with a particulate filter.  The coating contained 4,4 diphenyl methane diisocyanate which can cause allergic skin sensitization upon contact with the eyes and forehead.       Note:  Abatement certification is required for this item.

29 CFR 1910.134(d)(3)(ii):   The employer did not require a respirator that was appropriate for the chemical state and physical form of the contaminant:     (a)   Spray booth;  on or about August 22, 2012, and at time prior thereto, the employee spray a truck bed with Ameriguard coating wore only a half mask respirator with a particulate filter.  The coating contained 4,4 diphenyl methane diisocyanate which can penetrate air purifying filters and can cause allergic sensitization.       Note:  Abatement certification is required for this item.
