29 CFR 1904.7(b)(3):     The employer did not correctly record the following work-related injuries or illnesses on the OSHA Form 300 or equivalent.     Establishment: On or about 08/23/2012, the employer did not enter correctly record work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2012 in the following instances:    (a) Case number 6, 06/04/2012, Laborer:  an employee sprained his ankle resulting in days away from work whereby the employer did not record the number of days away in column (K).     (b) Case number 11, 08/23/2012, Laborer  an employee sustained burns on right side resulting in days away from work whereby the employer did not record the estimated number of days away in column (K).

29 CFR 1904.29(b)(1):     A log of all recordable work-related injuries and illnesses was not completed in detail as required by this standard:    Establishment: OSHA Form 300, Calendar year 2012  - The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300 in the following instances:    (a) Case #7, 06/11/2012, Laborer, Column (F):   where the injury was recorded as a sprained wrist the employer did not clearly identify the object/substance that directly injured the person and should have reflected the hand was caught between the truck and the grab bar.    (b) Case #8, 07/27/2012, Laborer, Column (F):   where the injury was recorded as a puncture wound to the hand the employer did not clearly identify the object/substance that directly injured the person and should have reflected puncture from a nail in the trash.    (c) Case #10, 08/22/2012, Laborer, Column (F):   where the injury was recorded as a left arm the employer did not clearly identify the object/substance that directly injured the person.    (d) Case #11, 08/23/2012, Laborer, Column (F):   where the injury was recorded as burns on right side the employer did not clearly identify the object/substance that directly injured the person and should have indicated sudden release of compressed air from a tire.
