29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses was not completed in detail as required by this standard: Establishment: OSHA Form 300, Calendar year 2012 - The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300 in the following instances: (a) Case #7, 06/11/2012, Laborer, Column (F): where the injury was recorded as a sprained wrist the employer did not clearly identify the object/substance that directly injured the person and should have reflected the hand was caught between the truck and the grab bar. (b) Case #8, 07/27/2012, Laborer, Column (F): where the injury was recorded as a puncture wound to the hand the employer did not clearly identify the object/substance that directly injured the person and should have reflected puncture from a nail in the trash. (c) Case #10, 08/22/2012, Laborer, Column (F): where the injury was recorded as a left arm the employer did not clearly identify the object/substance that directly injured the person. (d) Case #11, 08/23/2012, Laborer, Column (F): where the injury was recorded as burns on right side the employer did not clearly identify the object/substance that directly injured the person and should have indicated sudden release of compressed air from a tire.
