29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: A. R. G. Zachrich Construction, Inc. worksite located on Albon Rd. (North of Longworth), Holland, Ohio: On or about September 6, 2012, the employer did not ensure employees were protected from head injuries while working near the Komatsu PC200 hydraulic excavator. Due to this condition, employees were exposed to impact injuries caused by struck-by hazards.
