29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  The employer does not guard the point of operation and rotating parts of machines whose operation exposes employees to injury. This violation was observed on or about September 6, 2012, in the Production area where employees were exposed to struck-by and caught-in hazards when operating the CNC Mills number 204 and 205 and CNC Lathes numbers 105, 108, 109, 111, 115, 116, 117, 121 and 122 with the safety interlocks removed and no guarding of the point of operation and rotating parts.

29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel:  The employer does not ensure that all angle grinders are equipped with guards to protect employees from injury. This violation was observed on or about September 6, 2012, when an employee in the Production area was exposed to struck-by hazards while using a Makita, 9557PB, angle grinder without a guard.

This violation occurred on or about September 6, 2012, and at times prior thereto, when employees in the facility were required to handle hazardous chemicals before a written hazard communication program had been developed and implemented.
