29 CFR 1926.501(b)(2): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Jobsite: Employees moving and handling concrete forms on top of a retaining wall, and exposed to a fall of approximately 12 feet, were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems, for a jobsite located in Lincoln RI on or about 9-6-12. The employee did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicible tp his work environment to control or eliminate any hazards or other exposure to illness or injury. (a) Jobsite; Employees were not adequately instructed in the avoidance of unsafe conditions on the site.
