29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide. On or about September 5, 2012 on the east side of the construction site, an employee was engaged in cornice work on a residential building from job made pole scaffold platform that was not 18 inches wide exposing the employee to a fall of 16 feet to the ground below.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section. On or about September 5, 2012 on the east side of the construction site, an employee was engaged in cornice work on a residential building from job made scaffold without fall protection. The employee was exposed to the hazard of falling approximately 16 feet to the ground below.
