29 CFR 1926.102(a)(1):  Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    At the site, on September 5, 2012, employee was exposed to a struck-by hazard, as he worked on the roof of a residential structure, installing asphalt shingles with a pneumatic framing gun, without any form of eye protection.

29 CFR 1926.1053(b)(1):  When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:    At the site, on September 5, 2012, employee(s) were exposed to a fall of up to 12 feet, as the ladder side rails of the extension ladder extended less than 3 feet above the roof to which they were accessing.
