29 CFR 1926.451(c)(2):   Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    The employer does not provide the scaffold with base plates, mud sills or other adequate firm foundation.  This violation was observed on or about September 4, 2012, on the newly construction of a residential home where employees were exposed to a fall hazard when required to conduct brick work at a height of 13 feet without base plates, mud sills or firm foundation.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that supported poles, legs, posts, frames, or uprights are on base plates, mud sills or other adequate firm foundation.

29 CFR 1926.451(e)(1):   When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    The employer does not provide employees with access to the scaffold platform.  This violation was observed on or about September 4, 2012, on the newly construction of a residential home where employees were exposed to a fall hazard when accessing the platform at a height of 13 feet without a ladder.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that when scaffold platforms are 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders or similar surface is used.

29 CFR 1926.451(g)(4)(i):   Guardrail systems were not installed along all open sides and ends of platforms.    The employer does not install guard rails on the scaffold.  This violation was observed on or about September 4, 2012, on the newly construction of a residential home where employees were exposed to a fall hazard when required to conduct mansory work from a platform of 13 feet without a guard rail system.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that guardrail systems are install along open sides and ends of platforms.
