29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph. Note: This paragraph does not apply to large area scaffolds where the employer has satisfied the following criteria: the ladder was placed against a structure which was not a part of the scaffold and the scaffold was secured against the sideways thrust (a) Jobsite: On or about September 7, 2012, an employee was working from a 40 foot Werner aluminum extension ladder that was placed on top of a section of tubular welded frame scaffolding. This was done to increase the height of the ladder.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) Jobsite: On or about September 7, 2012, an employee came within approximately 12 inches of uninsulated, energized (120/240 v AC) overhead electric lines as he climbed and descended a 40 foot aluminum Werner extension ladder.
