29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards: On September 5, 2012, an employee was exposed to falls from a height of up to approximately 22 feet to the ground below, while he was decking the roof without having been effectively trained to recognize the need for fall protection.
