29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: a) FedEx Ground Package Systems Inc - Keasby, NJ - Hazardous Materials Quality Control Area: Employee utilizing employer provided 3M Model 8577 disposable P95 particulate respirator without employer providing information as required by 29 CFR 1910.134 Appendix D. Condition observed on or about September 7, 2012.
