29 CFR 1910.95(c)(1):   The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    Casting Department:  The molder operating the casting machine was exposed to noise at 124.9% of the permissible daily exposure (8-hour time weighted average of 80 dBA) or the equivalent sound level of approximately 91.6 dBA during a 396 minutes sampling period on 9/11/12: exposure calculation includes a zero increment for the 84 minutes not sampled.    Casting Department: The employer did not implement their Hearing Conservation Program.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
