29 CFR 1926.651(i)(3):  Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures:      a) Jobsite, 100 Block Rochelle Ave. - An employee was unclogging a pipe below an undermined section of the street, on the west side of an excavation.  The undermined section was not supported to prevent possible crushing hazards.   Observed on or about 9/6/12.     b) Jobsite, 100 Block Rochelle Ave. -  An employee was unclogging a pipe below an undermined section of the street, on the east side of an excavation.  The undermined section was not supported to prevent possible crushing hazards.  Observed on or about 9/6/12.     NOTE:    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.652(g)(2):  Additional requirement for shield systems used in trench excavations. Excavations of earth material to a level not greater than 2 feet (.61 m) below the bottom of a shield shall be permitted, but only if the shield is designed to resist the forces calculated for the full depth of the trench, and there are no indications while the trench is open of a possible loss of soil from behind or below the bottom of the shield:      a) Jobsite, 100 Block Rochelle Ave. - Employees were installing a sanitary main in an excavation.  The employees were inside a trench shield which had been installed 30" (2.6') above the base of the excavation, exposing the employees to multiple serious injuries.  Observed on or about 9/7/12.      NOTE:  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
