On September 6, 2012, employees who were working from a 4:12 pitch roof were not protected from falling more than 13-feet to the lower level.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On September 6, 2012, at least three employees, who were preforming roofing work, were ascending and descending from an extension ladder whose side rails did not extend at least 3-feet above the upper landing. Renauld Ortiz dba Ortiz Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 314184540, citation number 01, item number 002, and was affirmed as a final order on November 28, 2011, with a final abatement date of November 28, 2011 with respect to a workplace located at 100 N. Preston Road P.O. Box 1150, Prosper, TX 75078.
