29 CFR 1926.417(b): Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits can be energized: In the Lighting Control Panel 3 Room, in Panel 4LBA1, there were no tags on the breakers 9 and 11 that controlled the 277 volts to the lights being replaced in the west ticket counter check in area. The breakers were in the off position. The door on Panel 4LBA1 was not locked. The door to Lighting Control Panel 3 Room was not locked. In addition, the circuits were not rendered inoperative as a fuse or other circuit element for each phase conductor was not removed, or the circuit conductors were not disconnected.
