(a) In the cleaning building adjacent to the blast cabinet, a floor hole 50 inches wide by 94 inches in length by 37 inches deep was unguarded and unattended, exposing employees to a fall hazard.

29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    (a) In the cleaning room above the blast cabinet, open-sided platforms without continuous standard guard railings exposed employees to approximate 120 inch fall hazards when working from that level.     (b)  In the cleaning room adjacent to the blast cabinet, an open-sided landing platform over the pit did not have standard guard railings that would protect employees from 51 inch fall hazards.     (c)  In the cleaning room above the break room and/or restrooms, an open-sided mezzanine did not have standard guard railings that would protect the employees from approximate 114 inch fall hazards.
