29 CFR 1910.147(c)(4)(i):     Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.   55 ton room - On or about October 17, 2012, the written energy control procedures did not include step-by-step lockout procedures for the Walsh No. 1, No. 5 and other part revolution mechanical power presses. Employee(s) were thereby exposed to the hazards associated with an inadvertant startup of equipment.

29 CFR 1910.147(c)(6)(ii):     The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  55 ton room - On or about October 17, 2012, the employer had not documented through a written certification that periodic inspections had been performed on the energy control procedures.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  55 ton room - On or about September 7, 2012, employee(s) were required to hand-feed paper/chipboard into the feed roller of the Sheridan power press. The feed roller on the Sheridan power press was not provided with an adequate guard to protect employee(s) from crushing injuries.

29 CFR 1910.217(e)(1)(i):     It shall be the responsibility of the employer to establish and follow a program of periodic and regular inspections of his power presses to ensure that all their parts, auxiliary equipment, and safeguards are in a safe operating condition and adjustment. The employer shall maintain a certification record of inspections which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the power press that was inspected.  55 ton room - On or about October 17, 2012, a program of periodic and regular inspections of the Walsh No. 1, No. 5, and other presses was not developed and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment.
