29 CFR 1910.303(b)(2):     Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    East side of the well site:  On or about August 21, 2012, employees inspecting drilling collars and drilling pipe used electric hand tools supplied through a metal receptacle box that was not mounted.  The metal box was intended for fixed, mounted use only.  The box had three knockouts removed allowing access to the box interior.  This exposed the employees to the hazard of electrical shock.

29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    East side of the well site:  On or about August 21, 2012, employees conducting inspection of drilling collars and drilling pipe used electric hand tools that were attached to damaged cord sets.  This exposed the employees to the hazard of electrical shock.
