29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Maintenance Shop in the facility The employer is failing to ensure that the tongue guard of the bench grinder is properly adjusted to protect employees from laceration hazards. The grinder in question was not adjusted properly.

29 CFR 1910.215(d)(3): The contact surface(s) of wheel(s), blotter(s) or flange(s) on grinding machine(s) were not flat and free or foreign matter: Maintenance Shop in the facility The employer is failing to protect employees from struck by hazards due to the equipment not being properly maintained. The surface of the grinding wheels on the bench grinder are worn and grooved so the wheel is distorted from its original shape, instead of being square.
