OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees to struck-by hazards while moving loads. In the Production Area an employee was exposed to a struck-by, crushing hazard from a falling load while using a spreader bar, a below the hook lifting device, in conjunction with a underhung overhead hoist, to lift and flip a top roof panel for the trailer due to the lack of proof testing and the unknown rated load capacity of the spreader bar.

29 CFR 1910.213(d)(1): Each circular crosscut table saw shall be guarded by a hood which shall meet all the requirements of paragraph (c)(1) of this section for hoods for circular ripsaws. On or about September 17, 2012, in the Production Area an Assembler employee was exposed to an amputation hazard while cutting wood on the Craftsman 10 inch table saw machine without a guard over the saw blade.

In the Production Area on the Rockwell Belt Sander, an employee was exposed to a electric shock hazard from the electrical cord being pulled away from the fitting, creating strain on the terminal screws.
