29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) On September 05, 2012, at a jobsite located at 3814 Central St, Kansas City, MO., employees using a portable extension ladder were exposed to fall hazard injuries, in that the ladder did not extend 3 feet above the upper landing surface. Employees were exposed to a fall hazard of approx. 12 feet to floor level. Abatement certification for this violation is not required.

29 CFR 1926.1060(a)(1): The employer did not ensure that each employee using ladders had been trained by a competent person in the proper use and placement in handling the ladders: a) On September 05, 2012, at a jobsite located at 3814 Central St, Kansas City, MO., employees using portable extension ladders were exposed to serious injuries, in that the employees had not been trained in the proper placement and use of ladders used to access upper work areas of the job. Abatement certification is required for this item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
