29 CFR 1926.252(a):     An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):                          On or about 09/04/12, employees were exposed to a struck by hazard while employees working above, on the approximately 23' high roof, threw asphalt shingles off of  the roof and into a dump trailer.

29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about 09/04/12, employees were exposed to a fall from height hazard of approximately 23 feet to the concrete ground below, while removing and replacing asphalt shingles without fall protection.
