29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): (a) Colorado Concrete Company LLP at 150 Jasmine St, Denver, CO 80224: On and before September 6, 2012, the exposing employer did not ensure that each employee working next to and near the vertical side walls inside the basement excavation while installing concrete footings was protected from cave-in. The depth of the northwest side of the excavation was approximately 9 feet in depth. This condition exposed employees erecting concrete forms in the excavation to crushing hazards by excavation wall cave-in.
