29 CFR 1910.184(d): The employer did not ensure that each day before use, slings and all fastenings and attachments were inspected for damage or defects by a competent person designated by the employer. Damaged or defective slings were not immediately removed from service: (a) Mountain States Casing Company, LLC at Nitzel #1, County Rd 4: On and before September 7, 2012, the exposing employer (casing contractor) did not ensure that damaged slings were immediately removed from service, in that employees attached frayed and damaged wire rope slings to hoisting equipment to lift casing pipe from the catwalk to the rig floor. This condition exposed employees working on the rig floor to struck-by hazards as a result of the wire rope slings failing.
