29 CFR 1910.213(d)(1): Each circular crosscut table saw was not guarded by a hood: (a) 75 Appleton St Holyoke , MA Laminate Dept.: On or about August 2, 2012, an employee partially amputated three fingers on his left hand while using a Rigid brand 10 inch table saw that was not equipped with the manufacturer supplied blade guard at the time of the incident: Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1910.304(f)(1)(iv): Overcurrent devices were not readily accessible to each employee or authorized building management personnel: (a) shop floor (production) at 75 Appleton St Holyoke , MA : On or about August 2, 2012 the main circuit breaker panel, electrical power cut off switches and other circuit breaker panels (3 distinct locations) were blocked with cardboard boxes of shop material, a welding machine and moving blankets among other items preventing ready access to the devices: Conklin Office Services, Inc.LLC was previously cited for a violation of this occupational health and safety standard, 29 CFR 1910.304(f)(1)(iv) which was contained in OSHA inspection number 313208548 which became final order on March 15, 2011 with respect to a violation at 75 Appleton St in Holyoke, MA.
