29 CFR 1910.184(d):     Damaged or defective sling(s) were not immediately removed from service:    a)  Sheet Metal Area, 707 West Spring Garden Street, Palmyra, NJ:  A sling that was damaged was not removed from use.  The sling was used in the sheet metal area to move signs.  The sling was damaged and a knot had been tied in the sling in order to use the equipment, observed on or about 9/10/12.

29 CFR 1910.304(b)(3)(ii)(C)(3):     Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which were fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective was not removed from service until repaired;    a)  Welding Area, 707 West Spring Garden Street, Palmyra, NJ :  The electrical cord for the DeWalt sander had damage to the cords in multiple places.  The damaged cords were covered with electrical tape in some areas, observed on or about 9/10/12.      b) Sheet Metal Area, 707 West Spring Garden Street, Palmyra, NJ :  The electrical cord for the DeWalt sander had damage to the cords in multiple places.  The damaged cords had wires that were exposed, observed on or about 9/10/12.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
