Guardrail systems were not be installed before the scaffold was released for use by employees other than erection/dismantling crews. This violation occurred on September 11, 2012 at a workplace located at 2143 Joe Field Rd., Dallas, Texas 75229; where at least three (3) employees were observed working from scaffolding, more than 10-feet above ground level, without the use of guardrails or other fall protection.
